LONG-TERM CARE

Versa-Tags provides a $1,000 long term-care benefits to

ALL eligible employees with NO cost! You also have the

option to purchase (Buy-Up) additional long-term care

coverage. Please review the UNUM benefit information

for additional details.

EMPLOYER PAID BENEFIT:

■ $1,000 monthly benefit in long-term care facility or

professional home and community care

■ 3 years facility benefit duration

■ Elimination period of 90 days

■ Lifetime maximum of $36,000

EMPLOYEE OPTIONAL BUY-UP

■ 5% simple or compound inflation protection

■ Up to $9,000 facility monthly benefit in $1,000

increments

■ 6 Year or lifetime durations

■ Medical underwriting is required for any benefit

amounts over $6,000 or if lifetime duration is elected

ACCIDENT INSURANCE

Accident insurance is offered to you on a voluntary basis

through Colonial Life. This plan helps offset unexpected

medical expenses that result from off the job accident

related expenses. Accidents are usually followed by a

series of bills. Even if you have good insurance, you may

still have to cover out-of-pocket costs, such as:

Doctor bills

■ Ambulance fees

■ Hospital expenses

Insurance Company can help protect you, your spouse

and your dependent children from the unexpected

expenses of an accident.

You are paid benefits to help you with the care and

treatment of a covered accidental injury.

■ Your benefits are paid directly to you (unless you

specify otherwise).

■ You are paid benefits regardless of any other

insurance you may have with other insurance

INSURANCE

This benefit provides cash payments in the event of a

critical illness and is offered to you on a voluntary basis

through Colonial Life.
