Benefits Guide

Accidental Death & Dismemberment – AD&D (Cigna)

You can enroll in both Company-paid and voluntary employee-paid Accidental Death &

Dismemberment (AD&D) options. Your beneficiary receives a benefit equal to your

coverage amount upon your death.

Basic Employee AD&D (Cigna)

Your AD&D coverage provides a lump-sum benefit to you or your beneficiary if you or any covered

dependent dies or suffers a severe injury resulting from an accident. This costs you nothing and

provides an AD&D benefit of two times your annual base salary (rounded to the next higher

$1,000) to a maximum of $500,000.

If you’re over 65, please see the plan documents on StanNet for details about your benefits. Your

coverage is reduced at the beginning of the plan year when you reach age 70.

Voluntary Employee and Dependent AD&D (Cigna)

You may select additional AD&D coverage for yourself, your spouse or domestic partner, and your

children or your domestic partner’s children according to the following schedule.

Voluntary AD&D Coverage

AD&D and Personal Accident Insurance (PAI) benefit amount.

(AD&D for employees only; PAI for dependents only)

15% of employee amount per child
