WORKSITE BENEFIT
PROGRAM
We are excited to announce that UNUM will be our
Worksite Benefit carrier effective January 1, 2016. The
City of O’Fallon has taken the time and effort to
ensure the products being made available are of high
quality and competitively priced. We care about the
financial risks that our employees face. This City of
O’Fallon benefit program is intended to give you all
the information you need to determine if these
products can address some of the financial risks you
encounter.
Benefit Counselors can meet individually with you to
discuss your plan options and pricing for you and your
family. A signup sheet will be available at the health
fair on November 16
th
. You can schedule a time to
meet with a benefit counselor at the health fair or if
you wish to schedule a time now, please contact
Human Resources. What a perfect opportunity to
speak with a benefit counselor about your overall
benefits and take advantage of supplemental benefits!
The plans through UNUM have been designed to
provide dollars to offset new expenses and out of
pocket costs associated with accidents and/or
illnesses that can happen to anyone at any age. You
can use the benefit however you want – to help pay
medical bills or those everyday living expenses such
as housing, car payments, utility bills, child-care,
groceries, and credit card bills and even lost time from
work.
If you are currently enrolled in the Humana
Worksite plans, you may be eligible to convert
some of your coverages to a personal policy.
You will need to complete a Humana portability
form to do so, otherwise your coverage with
Humana will terminate. If you do not want to
convert your Humana policy or you are a new
enrollee, you can take advantage of the UNUM
Worksite plan offerings. You can elect coverage
through the CBAS enrollment site or meet with a
Benefit Counselor to discuss the plan options
which best fit your needs.
Below is a brief description of available plans. All plans
except disability are available for your spouse and children.
Please review this information and discuss with your family
as soon as possible.
Disability Income Insurance Protection
- Employees
may insure themselves so in the event an illness or injury
would cause an absence from work, a percentage of lost
income could be paid to you for necessary living expenses.
There are plan choices allowing you to tailor this important
benefit to your budget. The income received by you is
completely tax-free. If you are currently enrolled in the
disability plan through Humana your pre-existing condition
limitation will be waived.
Accident Expense Benefit
– Accident insurance is
designed to help meet the out of pocket expenses and
extra bills that can follow an accidental injury, whether
minor or catastrophic. This plan covers a wide range of on
and off the job accidental injuries. Indemnity lump sum
benefits are paid directly to the you based on the amount of
coverage listed in the schedule of benefits. The accident
plan is guaranteed issue, so no health questions are
required. This plan includes coverage for scholastic sports.
This coverage also pays $50 cash each year for any
insured that has 1 of 26 covered health screenings. This
plan is compatible with the QHDHP.
Critical Illness Insurance

Critical Illness insurance is
designed to help offset the final effects of a catastrophic
illness with a lump sum benefit if an insured is diagnosed
with a covered critical illness. This benefit is based on the
amount of coverage in effect on the date of diagnosis of a
critical illness or the date treatment is received according to
the terms and provisions of the policy. There are no
pre-existing condition limitations for you, your spouse and
children. This coverage also pays $50 cash each year for
any insured that has 1 of 26 covered health screenings.
This plan is compatible with the QHDHP. During this initial
enrollment period, you have a one-time opportunity to
enroll for $10,000 for yourself and $5,000 for your spouse
without needing to answer any medical questions!
Participation in any of the programs is completely
voluntary. However, if this is the first time these
benefits are being offered to you, we encourage you to
get with a benefit counselor to learn plan specifics,
obtain their own personal pricing and either enroll or
waive participation.
