On day 1 you should also give the following to
your Manager:
• Your Personal Details
• Emergency Contact Details
• Medical Questionnaire
• 2 References
• Proof of Legal Working (should have been received
at interview stage)
• Signed contract and if applicable

Opt Out Form
• Copy of P45 or P46
