Flexible Spending Accounts

Paychex remains the Flexible Spending Account

administrator for 2018. By enrolling in the FSA

you have the opportunity to set aside pre-tax

dollars for Health Care and/or Dependent Day

Care charges. As a reminder, up to $500 of your

unused FSA elections can be rolled over to the

next plan year, so please be prudent in your

annual election.

You MUST complete a Paychex enrollment

form to continue your Flexible Spending Ac-

count (Health Care or Dependent Care) for

2018.

Long-Term Disability

The Long Term Disability plan will continue to be

insured by The Standard. There are no changes

to the plan or rates for 2018.

Please see the disability portion of this guide for

plan details and the age banded Long Term

Disability rates. If you wish to elect coverage,

please complete both The Standard Enrollment

form and the Evidence of Insurability form.

LegalShield—Legal and Identity

Theft Protection Plans

LegalShield allows employees 24/7 access to

experienced independent attorneys and law firms as

well as licensed investigators skilled in identity theft

monitoring and restoration. See page 13 for more

information.

Health Advocate

The Health Advocate program offers employees

access to specialists who can assist with benefit and

claims questions. The Health Advocate program also

offers an Employee Assistance Program (EAP). See

page 14 for more information.
