To Search for Providers
Follow these easy steps to locate a doctor, hospital or
health facility participating with Anthem Blue Cross.
- Go to
- Click on Menu
- Select Member Needsunder Support
- Choose Find a Doctorunder Resources
- Enter your Member ID or User Name/Password
- Enter your search criteria
Follow these easy steps to locate a doctor, hospital, or
health facility participating with Kaiser Permanente.
- Go to
- Click on “Doctors & Locations”
- Enter Search Type, Region, Zip Code, and
Distance
- Click Submit
MEDICAL PLANS
Pacific Coast Title Company medical benefits are administered by Anthem Blue Cross/Kaiser Permanente.
Employees may select one of the plans (Kaiser HMO, Anthem HMO, Anthem PPO or Anthem Lumenos HIA) or
waive coverage altogether. Once you are enrolled, we suggest you register on either the Anthem Blue Cross
/ca or Kaiser Permanente ( to take advantage of the tools and resources
available to you.
For enrolled members
To register with Anthem Blue Cross:
1. Go to

3. Selection Registration
3. You’ll need to enter some basic personal
data (like your group ID, Date of Birth, First
and Last name) and then click Next
To register with Kaiser Permanente:
1. Go to
2. Click Registerin the top right corner
3. Select a language and click Continue
4. Answer “Tell us about yourself” questions and
then click Submit
