Why don’t (or won’t) we do this? Why aren’t we sharing the knowledge and wisdom of safety system

design and management? Even at an inter-departmental level, silos exist. In many of the major

incidents lack of information sharing was a significant contributor Visibility, safety achievement,

continued improvement and a sense of a wider team effort may be the answer.

If we could visibly see key performance indicators and how our efforts are improving those KPI’s

then maybe that breeds a sense of achievement and confidence that there is a safe working

environment. If this could then be compared between plants A & B and A is leading B. Then B learns

from A, what are they doing different that achieves better results. What about plant C & D or site 1 &

2 the processes may be different but with visibility of performance in real terms comes a sense of

healthy competition, a broader cooperation and a real knowledge sharing community.

We can see from

that there is no real change in major accidents within the period of 2000 to

2005 in countries where the Seveso Directive applies.. We could argue that there hasn’t been

sufficient time for regulated compliant projects and maintenance to take affect so let us look at the

period 2005 to 2012 which is as far as the records accurately report.
