YOUR BENEFITS

2016 Benefits Enrollment - What’s New

and Changing?

If you choose not to CHANGE your benefits this Annual

Benefits Enrollment period, you will continue in your

existing medical and vision benefit elections with NO

interruption of coverage. However, you will need to review

your other benefit options such as dental and voluntary

benefits because we are changing insurance carriers for

the 2016 plan year.

Medical

WCCI received a rate increase from Kaiser for 2016

which includes fees for Health Care Reform. WCCI’s

medical rates will be increasing starting February 1, 2016.

You will continue to have three medical plan options to

choose from: high HMO, low HMO and Multi-Choice.

Make sure you choose the right medical plan option for

your needs:

• The High HMO option only pays benefits when you visit

a Kaiser in-network physician or facility and you must

designate a Primary Care Physician (PCP)

• The Low HMO is similar to the HMO option but has

higher deductible and out of pocket maximums but lower

office visit copays

• The Multi-Choice option provides more flexibility. You

have the option to access benefits from three tiers of

providers: Kaiser network, PHCS network and any out-

of-network provider but the out of pocket costs varies

between the three tiers

A Gap Medical Plan through Assurant and AFLAC will

continue for 2016 with no change in premiums.

Dental

The Guardian has been chosen to be our dental provider

in 2016. There is a slight increase in premiums, but the

benefits no longer include a Missing Tooth Exclusion and

has improved network providers. If you were not previously

enrolled in the dental plan, you can enroll this year without

any waiting periods.

Vision

The vision plan will be offered through EyeMed. Private

practitioners will still be covered, but now you will have

benefits at major retail chains to include Pearl Vision,

LensCrafters, Target Optical and other regional retail

chains.

Basic Life/AD&D, Voluntary Life/

AD&D, Long Term Disability, Short Term

Disability, EAP, Travel Assistance and

Identity Theft

These plans will now be offered through Aetna Healthcare.

If you wish to increase your voluntary Life/AD&D benefit

amounts above the guarantee issue amount of $50,000,

you will be required to answer medical questions with your

application.

AFLAC

AFLAC plans will continue to be offered in 2016.

403(b) Savings Plan

There are no changes to the 403(b) plan. The maximum

contribution amount for 2016 will remain $18,000.

Checklist – What To Do

Use this checklist to help you make your benefit choices

for the 2016 plan year. The Annual Benefits Enrollment

period is December 29, 2015 through January 7, 2016.

Changes are effective February 1, 2016.

Review your 2016 Benefits Enrollment Statement

Review the 2016 Annual Benefits Enrollment Guide.

Decide what benefits you want to enroll in or change for

2016.

Confirm your dependents. Be sure to check your

dependent coverages and remove any ineligible

dependents from your coverages.

Questions about your benefit options? Schedule a

meeting with the Human Resources Benefits Team

If you wish to enroll in Voluntary Long-Term Disability or

enroll in or increase your Voluntary Life, you must

complete an evidence of insurability form and you must

answer medical questions.
