Basic Life and Accidental Death & Dismemberment

As an employee of Places for People, Inc. you are provided a $50,000* Basic Life and Accidental Death &

Dismemberment benefit. Effective January 1, 2016, this benefit will be administered by Anthem. (* An age

reduction schedule applies.)

Now is a great time to ensure that Places for People has the most up to date beneficiary information on file for

you. You will be asked for this information during the enrollment process outlined on page 12.

Places for People will continue to pay this benefit on your behalf and it equates to approximately $76 per

employee per year.

Voluntary Term Life

Places for People will continue to offer Voluntary Term Life insurance to all eligible employees effective

January 1, 2016, through Anthem. You must first purchase coverage on yourself in order to purchase

additional coverage for your dependents.

IMPORTANT!

If you did not enroll in the voluntary life program when you were first eligible and you have not

been previously declined for coverage, you can do so now for $10,000 of coverage without answering

medical questions! For any amount over $10,000 you will be required to completed a medical questionnaire.

If you elected at least $10,000 of additional coverage when you were

first eligible, you have the opportunity to increase your existing amount

of coverage by one increment without having to answer medical

questions, so long as you or your spouse have not been previously

declined for coverage. Your additional coverage will automatically be

approved.

Below is a reminder of the maximum coverage levels and Guarantee

Issue amounts.

EMPLOYEE COVERAGE

Employees may elect coverage in increments of $10,000 up to 5 x

Salary or a maximum of $500,000 whichever is less. The Guarantee

Issue amount for newly eligible employees is $150,000.

SPOUSE COVERAGE

Spousal coverage is available in $5,000 increments not to exceed 50%

of the employee amount up to a maximum of $250,000. The

Guarantee Issue amount for newly eligible spouses is $20,000.

CHILDREN

Coverage is available at a maximum of $10,000 for children who are 6

months and older. The monthly cost is $2.00 per $10,000 of coverage.

Age reductions apply to all employee and spouse benefit amounts

beginning at age 65. Spouse rates are based on the employee’s age.
