Voluntary ShortTerm Disability Plan Summary

Voluntary LongTerm Disability Plan Summary

Places for People will continue to offer this voluntary benefit to eligible employees through Lincoln Financial effective

January 1, 2016. Short-term disability is intended to protect your income for a short duration in case you become ill or

injured.

Beginning on the 31st day of an illness or injury you are eligible to receive 60% of your weekly income to a maximum

of $650. The benefit would last for a maximum of 9 weeks.

If you did not enroll when you were first eligible, you can do so now. You may not be eligible for benefits if you have

received treatment for a condition within the past 3 months until you have been covered under this plan for 12 months.

Places for People will continue to offer this voluntary benefit to eligible employees through Lincoln Financial effective

January 1, 2016. Long-term disability is intended to protect your income for a long duration after you have depleted

short-term disability or any sick leave you may have accumulated through your employer

After 90 days of disability you are eligible to receive 60% of your monthly income to a maximum of $4,000. Duration of

this benefit is based upon the extent of your disability and contract maximums.

If you did not enroll when you were first eligible, you can do so now but you will be required to complete an Evidence of

Insurability form and be approved for coverage.

and career, financial, and several other topics. As a reminder, Places for People pays for all

of the costs associated with this plan so that it is available to all employees and their family

members at no additional premium.

Retirement Services are offered by One America (formerly knows as AUL). Employees are able to

participate at any time during their employment and can decide the amount to contribute. Please
