Deponent - the manner in which
it came into the possession of the
Deponent to be stated.
(x) That a registered Medical
Practitioner has certified that the
Donor is, or is becoming,
incapable by reason of a mental
condition of managing his or her
own property and affairs.
(xi) That there is no other person to
whom notice is required to be
given under paragraph 2 of the
First Schedule to the Act.
(xii) Whether there is any reason to
believe that appropriate enquiries
might bring to light evidence on
which the Court could be
satisfied that one of the grounds
of objection set out in Section 10
(3) of the Act is established.

The following documentation (or so
much thereof as shall be in existence at
the time of the making of the
Application and as is in the possession
or procurement of the Deponent) should
be exhibited in the grounding Affidavit.
(i) The EPA.
(ii) Copy notice of Execution by the
Donor of the EPA given to the
first notice party.
(iii) Copy notice of Execution by the
Donor of the EPA given to the
second notice party.
(iv) Copy notice of Execution by the
Donor of the EPA given to any
the Donor.
(v) Copy notice of Intention to apply
for registration given to the
Donor.
(vi) Copy notice of intention to apply
for registration given to the first
notice party.
(vii) Copy notice of Intention to apply
for registration given to the
second party.
(viii) Copy notice of Intention to apply
for registration given to the
second notice party.
(ix) Affidavits of Service of the
Notices referred to at paragraphs
(v), (vi), (vii) and (viii).
(x) Certificate of a registered
Medical Practitioner to the effect
that the Donor is (or is becoming)
incapable by reason of a mental
condition of managing his/her
property and affairs.
The annual soccer match between the
Solicitors and Barristers took place
at Blackhall Place on Friday, 24
May. The Barristers turned out
their strongest team in several
years, including current and former
league of Ireland players in their
line up.
The home side suffered an early
blow with a controversially awarded
penalty box incident involving
Stephen Noonan. Aaron Shearer
for the Barristers converted.
Donagh Shaffrey equalised for the
Solicitors on 20 minutes from the
penalty shot. Another Shearer goal
followed, but Leo Fay equalised for
the Solicitors on the stroke of half
time to conclude a rubustly contested
first half.
The Solicitors upped the ante in the
| second half, with John Davis and
j Garbhain O'Nuallain raiding down
the flanks, Veterans, Tom Donaghy,
| Martin Moran and Mark Ronayne
! controlled play through the middle.
; Niall Clancy and Seamus Roe
competed physically with their
| opposites, and substitutes John
Kilroy and Declan Black also made
j their mark.
The Barristers buckled under the
I pressure and the Solicitors sailed
I home with two strikes fromMoran
i and one from Davis, leaving the
S Solicitors with a 5-2 victory, which
! was celebrated well into the Friday
! evening by players and supporters.
