G. Review all personal injury and property damage accidents involving City owned or leased
vehicles operated by a City employee to determine preventability or non-preventability
of the accident in order to assess points for driver’s review and to determine eligibility for
departmental awards.

6.1 Prior to each scheduled meeting of the Departmental/Divisional Safety Committee, the Safety
Committee Chair will prepare an agenda for the meeting including each item to be addressed at the
meeting. The Chair will provide each member of the Safety Committee with a copy of the agenda
prior to the meeting.
6.2 At the meeting the Safety Committee Secretary will take minutes. These minutes will include
a list of members present and absent at the meeting. Discussions where no action is taken will be
summarized. All decisions will be documented specifying the voting numbers or indicating the
decision was by acclimation.
6.3 The Secretary will prepare the minutes and forward to the Safety Committee Chair.
6.4 After a review by the Chair, the minutes and the meeting agenda will be sent to the appropriate
department/division head and a copy will be sent to the Safety and Health Manager.
