Navigate to the right side of the screen to the “Claim Association” section.

If this is the first time a claim has been filed, enter the following information:

Personal Identification Number (PIN)
b.
Jurisdiction Claim Number (JCN)
c.
Last Name
d.
Injury Date

Click the “Add” button.
