SUBMITTING CLAIM FOR BENEFITS/REQUEST FOR HEARING
This section covers the online submission of the Claim for Benefits. The Injured
Worker should submit a Claim for Benefits to the Commission as soon as
possible.
If you are requesting a hearing, you must file medical reports
supporting your request with the Commission.

Navigate to the right hand side of the screen to the “Claim Association” section.
Notice that the JCN does not have a Claim for Benefits associated and there is
an alert message.
