BELGIUM CONT’D

• Medical Certificate established by attending

physician certifying cause of death is not

any of the following contagious diseases,

plague, cholera, smallpox or typhoid.

Medical Certificate should also state

there are no hygienic or medico-legal

objections to transport of body

• Certificate established by funeral director

stating that body is embalmed according

to international shipping rules, coffin is

hermetically closed & meets all

requirements for international shipping of

human remains

• Upon presentation of above documents

Consulate General or Embassy will issue

“Laissez-passer” which should

accompany human remains, together

with Apostilled Death Certificate

• Ashes resulting from cremation of human

remains have to be shipped to Belgium

enclosed in hermetically sealed urn,

accompanied by Consular letter &

Apostilled Death Certificate

• Official Certificate of Cremation issued by

funeral home

• Statement drawn up by funeral director

certifying urn contains ashes of

deceased, that it is hermetically sealed &

ready for international transport

• Statement issued by Belgian city hall where

ashes will be transported, stating

Belgian commune has given

authorization for burial of urn or to strew

cremation ashes

• Upon presentation of these documents

Consulate General or Embassy will issue

Consular letter that should be kept with

urn to prevent Belgian customs from

opening it. You should also keep

Apostilled Death Certificate with urn

• Please note Belgian funeral home may

request additional documents, such as

certified translation of Death Certificate

deceased (if no valid passport exists,

please send copy of Birth Certificate &

Citizenship Certificate)

• Letter from funeral home stating flight

itinerary

• Notarized letter from funeral home director

certifying that remains have been

prepared according to international rules

& regulations concerning transport of

human/exhumed remains, that casket/

urn contains exclusively remains of

deceased. Remains must be place in

hermetically sealed & metal-lined casket.

Casket must be placed in air tray.

Cremated remains must be transported

in funeral urns which are sealed &

securely protected from damage by

cushioned packaging

• Consular fee for issuance of Laissex-Passer

in the amount of $36.00, plus additional

$20.00 postage fee (or a prepaid return

envelope)

• Uncremated remains: must be placed in

hermetically sealed & metal-lined casket.

Casket must be placed in air tray

• Cremated remains: must be transported in

funeral urns which are sealed & securely

protected from damage by cushioned

packaging
