Sun Life Family Health Center (SLFHC)
offers a comprehensive suite of benefits
to promote health and financial security
for you and your family. This booklet
provides you with a summary of your
benefits. Please review it carefully so
you can choose the coverage that’s right
for you.
Benefit Basics
As a SLFHC employee, you are eligible
for benefits if you work at least 30 hours
per week. Most of your benefits are
effective on the first day of the month
following
30 days
of employment.
You may enroll your eligible dependents
for coverage once you are eligible. Your
eligible dependents include:
• Your legal spouse
• Opposite sex domestic partner
• Your children up to age 26, or if
over 26, physically or mentally disabled and incapable of self support
Once your benefit elections become effective, they remain in effect until the end of the year.
You may only change coverage within 31 days of a qualified life event.
Qualified Life Events
In most cases, you may only make benefit changes during annual enrollment. However, if you
experience a qualifying event, an event that causes you or a covered dependent to gain or lose
eligibility for coverage, you may make changes within 31 days of the qualifying event. For a

The following are some examples of Qualifying Events:
• Marriage
• Adoption
• Legal separation
• Birth
• Divorce
• Death
• You, your spouse, or dependent starts or ends employment that affects eligibility for
benefits
