Short-Term Disability

• Covers 66.67% of your base weekly salary, up

to a maximum of $1,000

• Benefit begins day 1 for accident and after 7

days of continuous disability for sickness

• Benefits payable for 13 weeks

Long-Term Disability

• Covers 66.67% of your base monthly earnings,

to a maximum of $6,000 or $15,000 (see plan

summary for details)

• Benefit begins after 90 days of disability

• Benefits payable to SSNRA

Life insurance is an important part of your financial

security, especially if others depend on you for support.

Accidental Death & Dismemberment (AD&D) insurance

is designed to provide a benefit in the event of accidental

death or dismemberment.

For those employee whos base annual earnings

(before overtime) are les than $50,000, SLFHC provides

$50,000

Basic Life and AD&D Insurance to all eligible

employees at no cost to you. For those employees whose

base annual earnings (b fore ov rtim ) are greater than

$50,000, SLFHC provides

1X your earnings

in Basic Life

and AD&D Insurance to a m ximum of $300,000 for all

eligible employees. Limited tax liability may apply for Life

and AD&D insurance coverage amounts over $50,000.

The Life and AD&D coverage is offered through

Prudential.

Disability Insurance Coverage

The goal of the SLFHC’s Disability Insurance Plans is

to p ovide you with income replacement should you

become disabled and unable to work due to a non-work

related illness or injury. SLFHC provides eligible

employees with disability income benefits at

no cost to you!

The Disability coverage is offered through

Prudential.

Voluntary Supplemental Life Insurance Coverage

For additional protection and security for your family, SLFHC gives you the opportunity to purchase life insurance at affordable rates.

You pay the premiums with after-tax dollars through convenient payroll deductions.

Supplemental coverage can be purchased:

• Employee – In increments of $10,000, up to 7 times your annual salary (not to exceed $500,000). Guarantee Issue amount:

$100,000

• Spouse – In increments of $5,000, up to 50% of employee amount (not to exceed $250,000). Guarantee Issue amount: $20,000

• Child(ren) – In increments of $2,000 to a maximum of $10,000.

Unless you are a new hire, any amount for which you apply requires that an Evidence of Insurability (EOI) be completed.

The Life and AD&D coverage is offered through

Prudential.
