LegalShield Identity Theft Plan equips you with the

information and expertise you need to help prevent

theft and resolve issues related to identity theft. The

plan covers not only you and your spouse but can also

include any dependent under the age of 18 with the

addition of Safeguard for Minors.

403(b) Vesting

Vesting refers to your right of ownership to the money in

your account. Keep in mind you always own the

money you contribute to your account and all of the

interest earned on those contributions. However, you

earn the right to keep any money that SLFHC contributes

to your account and the earnings on those contributions

over time.

403(b) Retirement Savings Plan

The SLFHC 403(b) Retirement Savings Plan offers a convenient way to save

for your future through payroll deductions.

Eligibility

You will

automatically

be enrolled at a

5%

contribution level upon hire.

Unless you make a change, your contribution will automatically increase by

1% January 1st up to a maximum of 10%.

Employee Contributions

Contributions from your pay are made on a pre-tax basis, up to the IRS

annual limit. If you are 50 years of age or older (or if you will reach age 50

by the end of the year), you may make a catch-up contribution in addition

to the normal IRS annual limit.

The 2017 IRS annual limit for salary deferral is $18,000; plus $6,000 if 50 or

older and making ‘catch-up’ contributions.

Sun Life Family Health Center’s Matching Contribution

After completing one year of service in which you worked at least 1,000

hours, you are eligible to begin receiving matching contributions from

SLFHC.

The 403(b) match for 2017 is 100% of the first 1% contributed by

employees. Depending upon SLFHC performance, there may be an

additional discretionary match at year end to those participants active on

the last day of the Plan year.
