16.0 FUNCTIONAL SAFETY ASSESSMENT – STAGE 5 MODIFICATION

This assessment is to review the changes made by a modification to ensure that the SIS is not

compromised by the modification.

The FSA will address the following as a minimum:

The recommendations and actions arising from previous FSA have been

resolved and completed;

Review of the following;

o

Description of the modification;

o

Reason for the modification

o

Hazards which may be affected by the modification;

o

An analysis of the impact on functional safety as a result of the

proposed modification;

o

Approvals for the modification;

o

Test used to verify that the change was properly implemented and the

SIS performs as required.

Assess how far within the SIS lifecycle to go back and review the impact of

the modification;

o

LOPA

o

SRS

o

Design

o

Installation

o

Testing

o

Operation

o

Maintenance

Review the status of operating manuals and documentation in respect to the

implemented modification;

Plans or strategies for implementing further FSA’s are in place;

Attendance of competent personnel relevant to the required disciplines to participate in the

FSA.

Personnel assigned to this phase to be detailed on completition of stage 4 Review

Design Consultant.

Attendance of competent personnel relevant to the design of the SIS. Provide the FSA team

with all necessary documentation. Update this Management of Functional Safety Document.
