Benny Card FAQ’s

How does the

Benny

card factor in?

Your

Benny

card is loaded with the entire amount of your annual election so that you can begin using

it

on the first day of your plan year

. Simply pay for your medical expenses at the point of sale with

your card. You will not be paying out of pocket, therefore you will no longer need to fill out claim

forms and wait for reimbursement! Some receipts may still be required to substantiate the

transaction.

What is required to GET a Benny card?

Simply make an election and complete the

Benny

card participation section on your election form

(An e-mail account is required to receive a card). You will automatically receive your

Benny

card in

the mail to the address provided on your election form. If you already have a Benny card, it is valid

for 3 years and will be reloaded with your new election. New cards are automatically reissued during

the month they are set to expire.

Can I use my card to pay for over the counter medicine that my doctor suggested I take?

Beginning January 1, 2011, the cost of over-the-counter medicines or drugs cannot be reimbursed

from the account unless a written prescription is obtained. Therefore, purchasing over-the-counter

medicines or drugs will not be allowed using the Benny cards. You will need to submit a manual claim

along with the written doctors prescription for those items to be reimbursed. This change does not

affect insulin, even if purchased without a prescription, or other health care expenses such as medical

devices, eye glasses, contact lenses, co-pays and deductibles.

Items that are "merely beneficial" to

your general good health are not reimbursable, such as vitamins and supplements.

What if I go to a discount store or supermarket to buy my prescriptions and get my eye exams? How do

you know what I’m buying?

Our system is set up to flag any transactions that DO NOT correspond to your health plan co-pays.

Therefore, you may be asked to submit a receipt AFTER you made the purchase so that we can

substantiate your claim.

BE SURE TO SAVE ALL OF YOUR RECEIPTS! YOU WILL RECEIVE AN E-MAIL

NOTICE WHEN A RECEIPT IS REQUIRED.

What happens if my receipt shows I accidentally used the card for an ineligible expense?

Your FSA account can be used for eligible medical expenses ONLY and you are responsible for

reimbursing your account if the card is used either accidentally or intentionally for an ineligible

expense. You will be notified by your administrator if that should happen and your card may be

deactivated until your account is reimbursed.

What if I don’t owe anything at point of sale (doctor’s office) but get a bill later?

You can still use the card to pay the bill by writing your Benny card number on the invoice and

mailing it in, or by providing the Benny card information over the phone.

What if I no longer work for the company and I still have “CONTRIBUTED” money in my FSA?

You have 90 days from the end of your plan year, which would be your date of termination, to submit

claims for any eligible expenses that were incurred DURING EMPLOYMENT. Your card will be

deactivated upon termination but you can obtain your FSA reimbursement by submitting a manual

claim for services provided prior to your termination date.
