What was the problem?

Manufacturers certificate and FMEDA report stated that the

device was: -

Type B

SIL 2 with HFT = 0

In the FMEDA report a table of failure rates show

SFF = 81%

But BS EN 61508 Ed 2 shows that for Type B, SFF>90% is

required for SIL 2, HFT 0

How can they claim SIL 2 with HFT 0?

FMEDA report states

Radar Transmitters meet requirements of IEC61511

11.4.4 Prior Use

Justified by detailing data from field tracking database

and 100,000,000 hours

Report and justification was sent to the HSE Specialist

Inspector for consideration
