• Death Certificate clearly stating cause of

death. If cause of death is unknown,

permit will not be issued. In case of

infectious disease, autopsy report is

required

• ID document / Passport

• Embalming Certificate clearly stating in case

of: (a)non-infectious – that remains were

embalmed, sealed in an airtight container

& placed in sturdy non-transparent coffin

(b)infectious – that remains were

embalmed, placed in polythene bag,

sealed in an airtight container & placed in

sturdy non-transparent coffin

• If embalming cannot be done due to

religious reasons, please include

statement of alternative that will be used

to ensure safe transport of remains

• Written statement from medical practitioner

that remains will not constitute danger to

public health

• Non-infectious disease certificate

• Letter from family member requesting

importation. If deceased died while on

business for their employer & employer

will be handling transportation of

remains, letter from employer must be

attached explaining situation

• If documents are not in English, certified

translation must be attached

• Covering letter from South African

representative that includes (a)name of

deceased (b)date of death (c)cause of

death (d)country of death (e)place of

burial (f)full contact # including dialing

codes

• Please note Department of Health does not

deal with undertakers directly. Only

documents from Department of

International Relations & Cooperation or

South African representative will be

accepted. This includes transit through

South Africa. No importation permit

needed if remains are cremated. Death

Certificate, Cremation Certificate & copy

of deceased ID to accompany ashes at

all times, National Department of Health

requires 2 working days to issue

importation permit

needed in order to register death of

citizen of Spain in consular registry. We

will also provide information about

procedures to transfer mortal remains

into Spain, if that is family’s decision,

consulate will not be responsible for any

fees related to such transfer to Spain

• Consulate will issue certificate addressed to

captain of aircraft to be passed on to

“Administrador de Aduanas” in Spain

stating that all proper paperwork has

been submitted

Lanka unless such corpse has been

embalmed & enclosed in lead or zinc &

encased in wooden casket. Should be

hermetically sealed

• Certificate from health authority where death

occurred stating that corpse has been

embalmed or packed as required by No.

by registering authority of place where

death occurred

• Burial/Transit permit, certificate from

relevant authorities of place where death

occurred stating that removal of corpse

from place of death has been sanctioned

by said authorities.

• Certificate from Sri Lanka Embassy stating

coffin which such corpse is encased

contains only body of deceased

• Certificate from local health authority stating

that deceased did not die of

communicable disease

• Passport of deceased

• Coroner’s Certificate (if death was not due to

natural reasons)
