SERBIA & MONTENEGRO (202)332-0333

• To transport human remains to Republic of

Serbia it requires prior permission. In

order to secure such a permit /called

SPROVODNICA/, certain documents &

procedures are required

• If deceased was citizen of Republic of

Serbia, had valid Serbian passport, did

not die from any contagious disease,

Embassy of Republic of Serbia or

Consulate General of Republic of Serbia

in Chicago or New York, depending on in

which state deceased had lived, may

issue permit (SPROVODNICA) upon

presentation of following documents:

• Request of family for issuing of document

(Sprovodnica)

• Deceased’s valid Serbian passport. If

deceased did not have valid Serbian

passport, it will be necessary to present

certificate on Serbian Citizenship &

document serving as proof of identity (ID

card, driver’s license, old passport etc)

• Certificate of Death

• Certified statement by local health authority

that deceased did not die of any

contagious disease

• Certificate issued by funeral home stating

human remains were not previously

burned, that they were prepared for

transport according to international

regulations, placed in hermetically sealed

metal/zinc box

• Funeral home’s letter stating flight itinerary;

date & place of departure; date & place of

arrival; name of consignee, if any

• Money Order in the amount of $9.00

payable to Embassy of Republic of

Serbia as a fee for issuing document

• Return self addressed envelope with paid

postage for registered mail

• If deceased was not a citizen of Republic of

Serbia, it will be necessary to present all

above & certificate of pertinent authorities

of Serbia (i.e. municipal authorities,

cemetery) that plot has been reserved at

a specify cemetery for burial of deceased

(coffin, urn)

• If cause of death was plague or variola,

Embassy cannot issue document

(Sprovodnica) without approval of proper

authority of Republic of Serbia

by country where death occurred (copy of

English translation required if death

certificate is in ethnic languages) Cause

applicable IV. Coffin Export Permit

from country exporting body V. Air

Waybill (Air Consignment Note) if by air

• If application is by funeral director, Permit to

Cremate/Bury will be granted provided

next-of-kin gives letter of authorization for

funeral director to apply for permit

• Coffin Permit costs $10.00 Permit to Bury/

Cremate will be issued with coffin permit

at no extra charge

• Prior written approval has to be obtained

from National Environment Agency,

Environmental Health Department for

• Embassy of Slovenia can issue post-mortem

passport for deceased remains to be

transported to Slovenia. We recommend

check directly with airline on

requirements & list of documents needed

to transport (cremated) remains to

Slovenia. Sometimes they do not require

post-mortem passport

• Passport of deceased (if person had

passport)

• Death Certificate, authenticated with seal

Apostille. Seal is issued in Office of

Secretary of State. Cause of death has

to be clearly stated. Otherwise physician

must provide statement giving cause of

death

• Cremation Certificate if remains are

transferred in urn

• Burial/Transit Permit issued by responsible

authorities of US

• Confirmation from funeral home in Slovenia

that they will arrange burial

• Data on air carrier, flight #, date & port of

entry into Slovenia

• Name & passport # of person who will

accompany urn (if applicable)

• Address & phone # of relatives in Slovenia

who will arrange burial (if applicable)

• Consular fee in amount of $19.00 payable

only with money order addressed to

Embassy of Slovenia

• Prepaid express courier envelope (i.e. US

Postal Service UPS etc, must have

tracking #) addressed from Embassy to

your address to receive authenticated

documents back by mail

• If Slovenian citizen registration of death can

be done through Embassy or directly in

Slovenia. Submit original Death

Certificate with Apostille seal. Death

Certificate will have to be translated to

Slovenian language & translation

notarized either by Embassy or certified

court translator in Slovenian

that all state & local regulations have

been met

check, payable to Consulate General of

Contents of parcel should be clearly

stated on US customs tag. 55. No

permission is required to ship cremated

remains to Slovak Republic. It is

recommended that prior permission be

obtained from respective local authorities

in Slovakia to place cremated remains in

local cemetery
