participate, for Paraguay, usually takes

• Death Certificate issued by competent

authority in country where death took

• Accompanying Medical Certificate, stating

that deceased did not die from infectious

or contagious disease (Fitosanitario

Certificado) original, fee $55.00 Note:

This includes illnesses that may

contaminate atmosphere or environment,

elements for personal use, humors,

excretions & secretions, among others

• Embalming Certificate issued by accredited

funeral service company or official

authority from corresponding country

Note: If embalming process took place

48 hours after time of death, corpse will

be accepted by certifying that it was kept

in a cold chamber. Otherwise, corpse

must be submerged in formalin during

required time period in order to stop &

neutralize decomposition. The latter

process must be accredited by pertinent

legal medical authorities or local medical

authorities

that body was properly embalmed or

cremated, casket contains only body of

deceased (for human remains only)

(c)Certificate of Cremation issued by

crematory (for cremated remains only)

(d)Certificate of No Contagious Disease

issued by State Department of Health (for

human remains only) (e)Burial/Transit

Permit (for human remains only)

• 6 copies of the following: (a)itinerary to

include full flight details & mode of

transportation (b)deceased ‘s ID card or

passport (c)identification documents

(passport, driver’s license etc) of person

who will travel with urn (for cremated

remains only)

• Name of receiving funeral parlor in

Philippines (for human remains only)

• Self-addressed return envelope with

appropriate stamps, via US Postal

Service or private courier of choice,

preferably with tracking #, if Consular

Mortuary Certificate is to be mailed back

• Processing fee $50.00, if Filipino & $25.00 if

US citizen payable in money order, bank

draft, certified check or cashier’s check,

& cash when personally applying.

Personal check & credit cards are not

accepted. Payments are non-refundable

• Applicants residing in US territories or other

countries within jurisdiction of Philippine

Embassy should enclose a treasure’s,

manager’s or certified check drawn from

any US bank payable to courier of choice

in US dollars to cover cost of mailing &

corresponding self-addressed courier’s

address label. Personal checks are not

accepted

• If deceased is no longer a Filipino citizen

reduce number of photocopies by 2 for

requirements 2 & 3

• Philippine Embassy/Consulate General

assumes no responsibility for any delay

or loss in mail or while documents are in

custody of courier service. Applicant

should note tracking # of all envelopes

used & submitted

POLAND

human remains issued by appropriate

county executive (Starosta, Mayor) in

Poland – a fax copy is acceptable

(a)Death Certificate (original or certified

copy) (b)Notarized affidavit by funeral

director, stating that body was fully

prepared for shipment & placed in

hermetically sealed metal coffin or similar

container, that coffin contains only body,

clothing, lining (c)Certificate or affidavit

from local health authorities stating that

there was no communicable disease at

time of death, that cause of death of

individual to be shipped to Poland was

not a communicable disease

• Written permission for shipment & burial of

human ashes issued by appropriate

county executive (Starosta, Mayor) in

Poland – a fax copy is acceptable

(a)Death Certificate (original or certified

copy) (b)Certificate of Cremation

(c)Notarized affidavit by funeral director,

stating that ashes were placed in

hermetically sealed urn or similar

container & there is nothing but ashes

inside

• Burial/Transit permit for removal out of State

• Consul fee $66.00 (money order, certified or

teller’s check payable to “Embassy of

Poland”)

• One must supply name of person or

company responsible for shipment (e.g. a

family member, airline, etc.). A permit

will be issued when all necessary

documents are received
