• Certified copy of Death Certificate must

show cause of death from certifying

doctor

• Non- communicable disease letter

• Original (local) Burial/Transit permit

• Certificate of Taharah (ritual washing,

dressing & casketing of remains)

• An E-Shure or permit. Notice from Chevrah

Kadisha (burial society) in Israel in which

they MUST state: (a)they accept

responsibility to receive remains (b)they

must indicate name of cemetery or other

place of burial in Israel (c)that deceased

(you are shipping) has grave paid for in

full. Declaration that deceased is being

shipped in either Zeigler case or “Israel

Shipper” or some other generally

acceptable sealer, that deceased shall be

shipped & buried within 48 hours, only

gel packs shall be used (we have these

declarations)

• If deceased is Israeli citizen–you will have to

procure deceased’s passport & ID# (this

is similar to a social security #)

• Consulate fee $16.00 (subject to change)

• Application for permit to transfer body to

Israel

• If shipping to areas under control of

Palestinian Authority (PA), please call

Consul General for further instructions

form Death Certificates with Apostille &

translation into Italian (please be advised

that this office cannot register death in

Italy without said document) (b)woman’s

maiden name MUST appear

• Proof of citizenship (passport, green card,

naturalization certificate, certificate of

birth etc)

• Medical certificate on doctor’s or hospital’s

letterhead stating cause of death & non-

communicable disease. Doctor’s name &

surname must be legible. Apostille

required

• Certificate from Health Department stating

non-communicable diseases in area of

death. Apostille required

• Affidavit from funeral home director as per

attached form. Apostille required

• Data sheet for deceased on funeral home

letterhead stating following information

(a)exact name & surname (maiden name

for women) (b)exact date & place of birth,

including name of city/town & province

(c)airport of arrival in Italy – Milan or

Rome (d)exact city/town & province of

burial in Italy (e)address & phone # of

relative in US and/or Italy (f)address &

phone # of receiving funeral home

• Please be advised that 1 additional Death

Certificate will be requested at airport

• Shipment of human cremains (ashes): 2

long form Death Certificates with

Apostille & translation into Italian(only 1

certificate if person was not Italian

citizen)

• Cremation certificate. Apostille required

• Data sheet for deceased (see above)

• Photocopy of Italian passport, if deceased

person was Italian citizen.

• Urn containing cremains must be sealed &

placed inside wooden box. Box’s lid

must be sealed with 4 screws

• Statement by embalmer indicating manner &

method of preservation

• Human remains shall be subject to the

following measures: (a)thorough

washing with effective disinfectant,

disinfection of all orifices, packing of all

orifices with cotton saturated with

effective disinfectant, wrapping in sheet

saturated with effective disinfectant &

placing in impervious coffin or (b)proper

embalming (arterial & cavity &

encasement in plastic container which

has been sealed by heat, or by adhesive

material prior to placement in non-

impervious coffin). If cause of death was

from quarantinable disease, then it

should be placed in impervious coffin.

Impervious coffin must be hermetically

sealed

• Cremated remains: (a)Death Certificate

(b)Certificate of Cremation. Evidence

connecting Death Certificate with

importation must be produced
