6.3 The benefit eligible employee will decide whether or not to maintain the City’s health and

dental insurance during the deployment. If yes, the employee will be required to pay his portion of

the premiums by check or cash on a monthly basis. If no, the employee will notify the departmental

benefits assistant to change his insurance status.

6.4 The benefit eligible employee will need to review his beneficiary status for retirement and

deferred compensation (401a and 401k) and make any desired changes.

6.5 Supplemental Pay - In order for the benefit eligible employee to receive supplemental pay

during a military deployment in time of war, the following procedures must be followed:

A. The supervisor will furnish to the benefits specialist in human resources a copy of the

activation orders so that the benefit eligible employee will be authorized to receive the pay

supplement while he is deployed along with the employee’s full name, Lawson identification

number and date of departure.

B. The deployed benefit eligible employee will furnish to the benefits specialist in human

resources his military pay stubs and/or other documentation needed to determine pay received

while in active duty. This documentation must be furnished on a monthly basis. Payments

will be made by check and will be mailed to the employee’s home address unless alternative

arrangements have been approved.

6.6 Upon release from the military deployment, the employee will notify his supervisor of the date

for return to City employment as specified in Section 5.3 of this policy.

6.7 Upon the employee’s return to work, he will complete a leave request form for the time absent

due to the military deployment and attach a copy of his military discharge form DD-214. He should

also make any necessary adjustments to his health and dental insurance.

6.8 In order to receive service credit with the North Carolina Local Governmental Employees’

Retirement System, the employee will provide a copy of his military discharge form DD-214 as

specified by the retirement system.

6.9 Upon receipt of release orders from an active duty deployment in time of war, the employee

must schedule a return-to-work evaluation with City medical services if he was injured while

deployed. The exam components will be determined by the medical provider.
