notice that his services are not required

the fee payable shall be 25 per cent, of

the relevant Court fee above.

(b) Where

the Court

is held outside a

radius of five miles from the doctor's

centre of practice and he is not given a

minimum of four days notice that his

services are not required 50 per cent of

the relevant Court fee above.
