e. List of providers the Agent/Broker is associated with.

We are fortunate to have preferred level relationships with most national providers and vendors as well as many

regional and local vendors, entitling CBIZ to preferred and expedited underwriting, set-up and implementation, and

claims/issue resolution units dedicated to servicing CBIZ business clients on a priority basis. We are preferred

producers and sit on Agent Advisory Councils for several major carriers and vendors, including Aetna, Blue Cross

Blue Shield, Anthem, CIGNA and United Healthcare and have strong relationships with third party administrators

such as UMR and CoreSource.

Additionally, we have preferred pricing with multiple wellness vendors, including Bravo and open enrollment

technology that we have vetted based upon their ability to integrate data, assist with maintaining compliance and

engage employees.

This is particularly important when utilizing your current vendors to the full capacity or selecting a new wellness and

open enrollment vendor and we have already done the due diligence for this, allowing Unity to leverage our preferred

pricing and spend time instead selecting the best provider based upon your culture.

f. Expected communication responsibilities.

You have a significant investment in your employee benefits program and CBIZ wants to make sure that you are

getting the best return on your investment. Clear, effective communication is the key to success for any employee

benefit plan. Health plans, in particular, are expensive programs that require employee decision-making, so it’s

essential to give employees the information and tools they need to enroll and make the most of the program.

Communication Media and Materials

CBIZ offers a range of communication services, from basic enrollment support to customized communication

campaigns. Materials and media may include:

Web blast announcements;

Online summaries, links to carrier sites, and benefit bulletins;
