1. Benefits Programs
Which benefit(s) programs your firm is bidding to be an agent of record for.
CBIZ will market the Group Health and Welfare Benefit Program as directed including medical, dental, vision,
disability, life insurance and all other health and welfare benefits programs including the wellness program. CBIZ will
provide the benefit proposal specifications, identify insurer/service providers, and negotiate terms and conditions.
CBIZ will also include qualitative and quantitative analysis, provide network analysis, pooling point and
administrative costs, including our recommendation. We will endeavor to obtain at least two quotations for each line
of coverage. We will analyze all funding mechanisms, which will include fully insured and self-funded options.
The criteria we use to analyze each carrier’s proposal are as follows:

Wellness capabilities
Benefit plan design
Administration of carrier
Funding mechanism
Customer Service
